Policies are important to run clinic smoothly and have better relationship between Doctors and patients.
Digital policy

Maple Clinic is reserving 20% of appointments exclusively for online bookings to enhance service efficiency. We encourage patients to book online, as it is quick, convenient, and helps keep phone lines available for seniors who may require assistance. Additionally, our secure messaging system ensures prompt and safe communication
Appointment Policy

Please be advised that every physician strives to see patients at their scheduled appointment times. However, due to the complexity of medical cases or unforeseen circumstances, delays may occur. We kindly ask that you plan for an additional 30 to 60 minutes when attending your appointment.
Narcotics, Sleeping pills prescribing policy

Narcotic (opioid) medications (e.g., Percocet, Oxycocet, Oxycontin, Dilaudid, Morphine, Tylenol #3) are not prescribed for non-cancer patients except in rare and justified circumstances.

These medications entail significant risks, including side effects and potential addiction.

We generally do not prescribe narcotic medications, including cases where patients are already using them under another doctor’s care.

If prescribed, patients must sign a contract and undergo regular drug testing.

Breach of the contract may result in discharge from the practice.
No show and cancellation policy

Missed appointments cause significant disruptions, including:

Wasted time for the doctor assigned to see you.
-
The efforts of team members to prepare for your visit go unused.

Other patients in need are unable to schedule appointments.
To help mitigate these issues:

Please cancel appointments at least 24-48 hours in advance to allow the spot to be offered to another patient.

The missed appointments will incur the following changes:

$50.00 for regular visits

$70.00 for longer visits like physicals, Pap smears, and counseling

Lateness Policy

To be respectful of patients’ time, you may be asked to reschedule your appointment if you are late more than 10 minutes.
Forms

Forms from third parties, such as insurance, must be brought in person and discussed with the doctor. Some forms require the doctor’s opinion or disclosure.

Most tax disability forms requested by patients are not applicable, as patients often do not qualify for tax disability.

Forms should not be sent via email.

Forms are not urgent. Some, like driver’s license or diet forms, may be completed the same day, but others requiring chart review may take up to 2 weeks.

Forms required by the Ministry of Social Affairs or other specific bodies may have varying charges. Discuss these directly with your doctor, not with the front staff.
Lab Policy

Please note the following regarding your lab results, X-ray, and ultrasound imaging:

If your results are normal, the clinic will not contact you.

If you wish to discuss your results, you may book an appointment to consult with the doctor.

If there are any concerns with your results, the clinic will contact you to discuss them.

Continuity of care policy

To ensure better continuity of care, patients will only be allowed to book appointments with their designated Family Doctor. Appointments can be made by calling ahead or booking online.

Please confirm the correct clinic location when scheduling your visit—either Maple at Markham or Bellamy location.

If your Family Doctor is on vacation and you have an urgent medical condition, please contact us. We will do our best to accommodate you by rescheduling with another Family Physician at one of our two clinics.
Behaviour & Conduct

Respectful Behavior: Staff must be treated with respect. Rude, disruptive, or aggressive behavior may lead to removal from the facility.

Children must always be supervised by a parent or guardian while in the clinic. The clinic does not assume responsibility for any injuries sustained on its premises. Additionally, any damage to clinic property will be charged to the parent or guardian responsible.

Infection Control: No food or drink is permitted in exam rooms.

Cell Phone Use: Refrain from using cell phones in exam rooms to ensure proper attention is given.

Injection Monitoring: After receiving injections, patients are required to wait 20 minutes in the clinic to monitor potential reactions.

Pet Policy: Pets are not allowed in the clinic, except for guide dogs assisting the visually impaired.

Privacy Protection: Pictures and videos are not allowed on clinic premises. Doctors may document exam findings with consent using secure medical records.

Scent-Free Environment: The clinic is a scent-free space to protect individuals with allergies.
